2013 College Freshman Scholarship
Application Form
OCJAA is sponsoring up to three (3) scholarships of $1,000.00 each to full-time
high school seniors who will be attending an accredited college/university as a freshman for the 2013-2014 academic year.
The scholarships will be awarded on July 4, 2013 at the OCJAA Annual Golf Tournament and Awards Luncheon being held
at Green River Golf Club. ELIGIBILITY: Any student of Japanese, ancestry either residing in Orange County
or graduating from a high school located in Orange County, are eligible to apply. Applications and corresponding
documents may be submitted by e-mail or postal mail and must be sent/post-marked by June 1, 2013.
A. SCHOLASTIC HONORS: On a separate
sheet of paper, list academic awards and/or honors and document the year in which they were received. Also, attach (1) Copy of your transcript through the
fall semester of your senior year (2) Copy of the Official College Board SAT scores. Any application missing one or both of
these will render your application invalid and will not be accepted for consideration.
B. ON-CAMPUS/OFF-CAMPUS ACTIVITIES: List your major on-campus/off-campus
activities. Identify leadership positions held, awards received with the appropriate year noted.
C. COMMUNITY INVOLVEMENT: List all community involvement. Identify specific
activities, level of involvement and leadership position if applicable along with the year(s) in which engaged. Your
participation in the Nikkei community will be given extra preference.
D. INTRODUCTION & ESSAY: Provide a one paragraph introduction of yourself. Also
submit a short essay on what your Japanese heritage means to you and if you intend to be involved in the Nikkei
community in the future.
AGREEMENT: I have read and fully understand the eligibility requirements and the
information required for a valid OCJAA scholarship application submission. I agree to verify my college
enrollment and substantiate any information I have provided. If selected, I understand that I must personally attend the
scholarship luncheon on July 4, 2013 at the
Green River Golf Club to receive the award.
